Look what "FND_UNSUCCESSFUL_LOGINS shows question mark in the TERMINAL_ID column (Doc ID 1496148.1)" says;

The column TERMINAL_ID contains only "?", as explained with
Note <751658.1> "How To Get The terminal ID For The Machine From Which A User Is Logged To E-Business Suite Applications" ...

Seems that first the table FND_UNSUCCESSFUL_LOGINS being updated, and then ( maybe with short delay ) an entry within access_log appears, which contains : errCode=FND_APPL_LOGIN_FAILED.

To get the IP address, tie up the data from FND_UNSUCCESSFUL_LOGINS table to the access_log, verify for for entries with "errCode=FND_APPL_LOGIN_FAILED", which should show the IP.
