EBS 12.2 Forms not coming up. Error not connecting to DB

EBS 12.2 App console comes up fine.
When I click on System Administrator/Request/Run
Forms start to come but but it  gives an error

Error
"Cannot complete  applications login. You may have entered an invalid applications password, or there may have been a database connect error"

Tested and validated guest password which is the default.
I can complete the adop cycle testing all passwords apps/system/weblogic
From weblogic console I tested the oacore server and forms server connections to database successfully.
checked context files for applsyspub password and also in sid.dbc file
I  tested all passwords successfully apps/applsys/applsyspub accounts by login in through sqlplus

Recently someone changed the apps password and I could not log to EBS console.
I reset apps/applsys/applsyspub/sysadmin passwords to default as per a MOS article from back-end
Reset using ALTER command and also updated encrypted password.
reran  the auto config on db nodes and app nodes successfully

I can login to EBS console and do all but I cannot run forms
I can also login to weblogic console.  

Any suggestions /solutions , I will appreciate any help.

Probably, there is an APPS_password inconsistency there.

Reset it step by step by using the exact instructions as documented.
Reset the password by following the document named -> How to Change Applications Passwords Using Applications Schema Password Change Utility (FNDCPASS or AFPASSWD)? (Doc ID 437260.1)

Don't forget to execute the steps documented under the subsection named -> "For 12.2 only:"
